River Paws – Services Agreement

Consent for Treatment and Release of Liability Agreement

Consent for Treatment Contract: As the owner of a dog that uses the services of River Paws LLC, I understand that River Paws LLC and its employees will exercise due care to protect the health and safety of my dog while in their care. If my dog sustains an injury, I authorize River Paws LLC and its representatives to obtain medical treatment. I authorize the attending veterinarian to start treatment in the event of an illness or accident. I will assume all costs related to the treatments deemed necessary by the veterinarian to provide care to my dog or the dog(s) my dog injured.

In the unlikely event that my dog passes away for any reason at daycare, I understand that he/she will be transported to my vet. I understand that every effort will be made to contact me at such an event. If I and my veterinarian are unavailable, my pet will be transported to UW Veterinary Care.

Waiver of liability statement: I understand the concept of doggy daycare is to allow dogs to be socialized by interacting with people and other dogs. As always, with the interaction of dogs, there is a chance of injury. I assume all risk of injury to my dog while at River Paws LLC, so long as reasonable care has been taken to prevent unnecessary injury.

I understand all the risks inherent in dog park activities, including but not limited to disease, bug/insect bites or stings, drowning, broken glass, transportation, and other injuries.  I agree that River Paws LLC is not responsible for any outdoor activity-related risks.

I agree that River Paws LLC is not responsible for any bathing and grooming-related injuries and waive and release River Paws LLC and its employees from any and all liability.

I understand the training River Paws LLC provides is an attempt to better my dog(s) behavior. I agree that River Paws LLC is not responsible for the behavior of my dog in any way and waive and release River Paws LLC and its’ employees from any and all liability related to the actions of my dog(s).

I hereby waive and release River Paws LLC and its’ employees from any and all liability of any nature for any injury, death, or the loss of my dog resulting from River Paws LLC actions or from the action of my dog or any other dog while in the custody of, or on the grounds or surrounding areas (Cherokee Dog Park) of River Paws LLC, or when using River Paws LLC services not resulting in negligence of River Paws LLC.

If my dog causes injury to another dog or to a person, I agree to indemnify River Paws LLC from any action that may be brought against it and for any defense, settlement, or judgment entered against River Paws LLC.

I will assume all liability for the actions of my dog and agree to abide by all policies and procedures of River Paws LLC. Each time I use the services of River Paws LLC, I affirm the terms of this agreement and the truthfulness and accuracy of all information provided to River Paws LLC.

Last-Minute Cancellation Fee: If you need to cancel a grooming appointment with River Paws within 24 hours or less of the scheduled date and time, a cancellation fee equivalent to 50% of the expected bill will be applied.
How to Cancel: To cancel a booking or appointment, please contact us as soon as possible by calling 608-571-7297 or emailing riverpawsgrooming@gmail.com. We understand that unforeseen circumstances may arise, and we appreciate your prompt notification.
Reasonable Exceptions: We understand that emergencies and unexpected situations may occur. In such cases, we will consider exceptions to the cancellation policy on a case-by-case basis. Please contact us immediately to discuss your circumstances.
Refund of Cancellation Fees: Cancellation fees are non-refundable unless we make an exception due to extraordinary circumstances.
-$15 Late Fee for being 10 minutes late or more. At this time it is the groomer’s choice if they can still take the dog depending on how late you arrive
-$15 Boarding if your dog is staying 2 or more hours past the completed time of the groom. Please let us know ahead of time if you need your dog to stay late.
